Have one of our preferred doctors or nurse practitioners assess your health, review your medications, and answer any questions you may have.
Take the first step and call 1-800-607-2348 (TTY 711)* to complete your required consultation and schedule your LiveHealthy Visit. Or log into the Member Portal, click the LiveHealthy Rewards button, and complete your LiveHealthy Visit consultation online.
Preventive Care: Up to $50 Reward Dollars
Earn reward dollars for these preventive care services (once each year):
- Flu shot ($10 in reward dollars)
- A1C test ($20 in reward dollars)
- Retinal eye exam with an EyeQuest provider
($20 in reward dollars)
Get Active: Up to $120 Reward Dollars
Earn $30 in reward dollars each quarter for doing one of these activities:
- Attend a One Pass® gym or class (virtual or in-person)
- Attend a Clover member event (virtual or in-person)
- Complete select poll(s) that are available on the Member Portal
- Register and log in to the LiveHealthy app (this reward can only be redeemed once)

What's the difference between the OTC benefit and LiveHealthy Rewards program?
Lets members earn reward dollars by completing activities that promote good health. You can even do some activities without leaving your home. And you can spend your reward dollars on just about any products or services! †
Gives members an allowance each quarter that allows you to purchase commonly used healthy items such as vitamins, band-aids, toothpaste, and aspirin.
How much money?
Up to $400 reward dollars per year.
$50–$180 per quarter
(Up to $200–$720 per year)
The amount varies by plan.
Are there any time limits to use the funds?
No quarterly restrictions
Must use within allocated quarter
(Jan.-March, April-June, July-Sept., Oct.-Dec.)
When are the funds loaded onto the card?
Earned reward dollars are loaded after completion of the reward requirement. (Load time may vary based on verification of reward completion.)
OTC dollars will be loaded onto the Flex Card at the beginning of each quarter.
Is there a limit on what I can use the card on?
No, you can use reward dollars on most items (grocery, transportation, etc.) except alcohol, tobacco and firearms.†
Yes, you must purchase OTC items from an approved list.
What if I spend more than what is on the card?
If you exceed your reward dollars on the card, you will be responsible for overage.
If you go over your quarterly amount and have reward dollars on the card, the overage will be covered by your reward dollars. If you do not have the reward dollars, you will be responsible for the overage.
Can the funds be converted to Cash?

†This reward can only be earned one-time only during your entire time as a member and will be earned on 1/1/2027 for activities completed in 2026.
††If your Clover plan coverage ends, any unused rewards will expire 90 days after your coverage termination date. Rewards earned for activities completed in 2026 or earlier will expire on June 30, 2027, even if you are still enrolled. If both expiration dates apply, your rewards will expire on the earlier date. Rewards earned for activities completed in 2027 will expire on June 30, 2028. Please use your rewards before they expire.
