- Enter your Member ID, birth date, and ZIP code
- Click ‘Next’ at the bottom of the page
- Create a username and password
- Add two security questions and answers

Your LiveHealthy Visa Flex Plus card is for both your Over-the-Counter (OTC) allowance and LiveHealthy Rewards. It’s a smart card that will first automatically deduct approved items from your OTC wallet if shopping at an approved retailer and then will take the balance from available LiveHealthy reward dollars.
The OTC allowance can only be used towards approved items with authorized retailers (see “How can I use my OTC allowance?” to find them).
LiveHealthy reward dollars are more flexible and can be used almost anywhere Visa is accepted* for almost anything, including groceries.
You can check your balance by logging into My Clover Member Portal and navigating to the Healthy Benefits+ site via “View My OTC Balance”.
Over-the-Counter (OTC) Allowance
Your OTC allowance balance resets every three months (January 1, April 1, July 1, October 1). Your quarterly allowance amount depends on your plan.
LiveHealthy Rewards
Members can earn up to $400 reward dollars* each year by completing healthy activities. See LiveHealthy page for more details.
You can shop with your OTC allowance in three easy ways.
- Shop in store. Log into My Clover Member Portal and navigate to the Healthy Benefits+ site via “View my OTC Balance” to find both participating stores and approved OTC items. There are over 40 national retailers available including popular chains, plus local stores.
- Order online. Shop with Healthy Benefits+ via My Clover Member Portal or add your LiveHealthy Card to your walmart.com account.
- Shop via Phone. Call 1-800-607-2348 (TTY 711) to place your order.
Your One Pass® benefit gives you free access to gyms and fitness classes through the Premium Network. You can access as many gyms as you’d like that are in the network. Go to youronepass.com to get your One Pass® member code and find gyms in the Premium Network.
You’ll complete and submit a consent for PHI sharing form. This allows Clover to share your Protected Health Information (PHI) with them.

†You can earn a maximum of $30 in reward dollars per quarter by completing "get active" activities.
** $30 LiveHealthy rewards per quarter for one of the following activities: Attend a One Pass® gym or class (virtual or in-person), attend a Clover member event (virtual or in-person), complete select poll(s) that are available on the Member Portal, register and log in to the LiveHealthy app (this reward can only be earned one time in 2026).
