Earn Reward Dollars Through Health-Related Activities
When you enroll in a Clover Medicare Advantage plan, you get a LiveHealthy Flex Plus Visa card. We add reward dollars to your card as you complete each activity. Reward dollars can be spent on groceries, clothes, and more.

Get $150 in reward dollars for completing a LiveHealthy Visit.
Have one of our preferred doctors or nurse practitioners assess your health, review your medications, and answer any questions you may have. It'll be good for your health—and your rewards balance.
Call Member Services at 1-800-607-2348 (TTY 711)* to schedule your LiveHealthy Visit.
Get up to $50 in reward dollars for preventive care.
We’ll reward you for completing certain preventive care services:
- Flu shot ($10 in reward dollars)
- A1C test ($20 in reward dollars)
- Retinal eye exam with an EyeQuest provider ($20 in reward dollars)
Each of these rewards can only be earned once in 2024.
Get up to $100 in reward dollars just for getting active.
We’ll reward you $25 in reward dollars per quarter for completing one of the following activities:
- Attend a SilverSneakers gym or class (virtual or in-person)
- Attend a Clover member event (virtual or in-person)
- Complete select poll(s)
- Creating a member portal account or logging in to your member portal account in 2024 (please note: this reward can only be earned one time in 2024)
Get up to $120–$300 a year for OTC items!
Every quarter, we automatically add $30–$75 to your LiveHealthy Flex Plus Visa card for you to buy over-the-counter items, such as pain relievers, vitamins, cold and flu medications, and more. The amount varies by plan.

What's the difference between the OTC benefit and LiveHealthy Rewards program?
Gives members an allowance each quarter that allows you to purchase commonly used healthy items such as vitamins, band-aids, toothpaste, and aspirin.
Lets members earn reward dollars by completing activities that promote good health. You can even do some activities without leaving your home. And you can spend your reward dollars on just about any products or services! †
How much money?
$30–$75 per quarter
(Up to $120–$300 per year)
The amount varies by plan.

Are there any time limits to use the funds?
Must use within allocated quarter
(Jan.-March, April-June, July-Sept., Oct.-Dec.)

OTC dollars will be loaded onto the Flex Card at the beginning of each quarter.
Earned reward dollars are loaded after completion of the reward requirement. (Load time may vary based on verification of reward completion.)
Is there a limit on what I can use the card on?
Yes, you must purchase OTC items from an approved list.
No, you can use Reward dollars on most items (grocery, transportation, etc.) except alcohol, tobacco and firearms.†
What if I spend more than what is on the card?
If you go over your quarterly amount and have Rewards dollars on the card, the overage will be covered by your Rewards dollars. If you do not have the Reward dollars, you will be responsible for the overage.
If you exceed your Rewards dollars on the card, you will be responsible for overage.
