Care at Home:
Our specialized doctors, nurse practitioners, and care team members can visit you in person or virtually, offering support for chronic conditions or serious illness.
When We Help

Clover Care Visits
We also offer something special called a Clover Care Visit. It’s a personalized health check-up, and it’s available at no additional cost to you. You can have this visit at home or online, whatever works best for you.
Here’s what you can expect during a Clover Care Visit:
- A specialized nurse practitioner checking your health
- Medication and wellness review
- Answers to your health questions
- Connection to helpful resources
- Flexible appointments (home or telehealth)

Welcome Home Clover Care Visits
Just got out of the hospital or a nursing facility? We can visit you at home to support your recovery. We’ll check your medications, and help you avoid another hospital trip. No extra cost. Just extra care.
In-Home Care for Serious Illness
If you’re dealing with a chronic or serious condition, our in-home care team can help. We provide extra support for you and your loved ones, helping you stay comfortable and safe at home.
Am I Eligible?
Good news! All Clover members qualify for at least one in-home or virtual Clover Care Visit per year. Plus, you’ll get a Welcome Home Visit after a hospital stay. You may also qualify for more services based on your health needs.
