Vaccine Resources

Flu vaccines are an important part of preventive care for Clover Health members. We’ve gathered some information on vaccine resources, particularly for homebound patients in states where Clover plans are offered. The goal is to give you information you need while encouraging vaccines for our members.

State	Resource
Alabama	Use 211 or see if your county’s health department has resources for vaccines for homebound patients.
Georgia	If a patient is unable to leave home to receive a COVID-19 vaccine, call 888-572-0112 to request a vaccine at home. For flu vaccines, call the Health Department Vaccine Scheduling Line at 888-457-0186 to see what resources are available.
Mississippi	Use 211 or see if your county’s health department has resources for vaccines for homebound patients.
New Jersey	If a patient is unable to leave home to receive a COVID-19 vaccine, a patient, healthcare provider, or family caregiver can request an in-home vaccination for the homebound individual here or call 855-568-0545. Use 211 or see if your county’s health department has resources for flu vaccines for homebound patients.
Pennsylvania	Call the PACE Call Center at 800-424-4326 for information on vaccines for homebound individuals or visit the Department of Health website.
South Carolina	Call 866-365-8110 for information on vaccines for homebound individuals.
